The one-sided form on dense paper. The size of certificate is ≈ 13,9 cm x 20 cm, with watermarks in the form of waves. All inscriptions are made in Russian and Chuvash languages. The certificate contains information about dead person‘s name and surname, date of death, age, a cause of death, the place of death and a place of death registration.
