The one-sided form on dense paper. The size of certificate is ≈ 14,2 cm x 20,2 cm, with watermarks in the form of waves. All inscriptions are made in Russian and Tatar (Kazan) languages. The certificate contains information about dead person‘s name and surname, date of death, age, a cause of death, the place of death and a place of death registration. In the bottom of the certificate are specified: National language, Goznak (State Insignia) and the year of issue.
