Form of the Death certificate, type 1970-1991:

The one-sided form on dense paper. The size of certificate is ≈ 15,2 cm x 21 cm, with the watermark in form of CCCP (USSR) inscription. All inscriptions are made in Russian language. The certificate contains information about dead person‘s name and surname, date of death, age, a cause of death, the place of death and a place of death registration. In the bottom of the certificate are specified: Goznak (State Insignia) and the year of issue.
