Form of the Death certificate, type 1970-1991:

The one-sided form on dense paper. The size of certificate is ≈ 15,2 cm x 21 cm, with the watermark in form of CCCP (USSR) inscription. All inscriptions are made in Russian and Finnish (Karelian) languages. The certificate contains information about dead person‘s name and surname, date of death, age, a cause of death, the place of death and a place of death registration. In the bottom of the certificate are specified: National language, Goznak (State Insignia) and the year of issue.
