The one-sided form on dense paper. The size of certificate is ≈ 15 cm x 21 cm, with the watermark in form of CCCP (USSR) inscription. All inscriptions are made in Russian and Buryat languages. The certificate contains information about the adopted child, child's birth name and birth surname, date of birth, place of birth, date of adoption of the child, name und surname after the adoption, information about adoptive parents and a place of certificate registration. In the bottom of the certificate are specified: National language, Goznak (State Insignia) and the year of issue.
