Form of the Certificate of acknowledgement of paternity, type 1998-2018:

The two-sided form of yellow colour on thick paper. The size of certificate is ≈ 18 cm x 25 cm, with the watermark in form of the State emblems of the Russian Federation. All inscriptions are made in Russian and Yakut languages. The certificate contains information about the child's father, child's name and surname, child's date of birth, information about child's mother and a place of certificate registration. In the bottom of the certificate are specified: National language, Goznak (State Insignia) and the year of issue.
