Form of the Birth certificate, type 1917-1927:

The one-sided form on thin paper. The size of certificate is ≈ 19,8 cm x 29,5 cm, without watermarks. All inscriptions are made in Russian and Ukrainian languages. The certificate contains information about child's name, birth date, parents' names, birthplace and a place of registration.
